Building a form

The builder works in blocks: short and long text, single and multiple choice, yes/no, dates, numbers with units, consent checkboxes, signatures, section headings — even a body map for marking treatment areas.

Two collection modes (per form)

  • Email after booking — the customer gets a secure link to complete before their visit.
  • Fill at booking — the form appears inside the online booking flow itself, so the booking can't complete without it.

Attach a form to a service and every booking of that service collects it automatically. Anything outstanding is flagged at check-in.

Privacy controls

Forms can be private to specific staff — a clinical form only the practitioner can read stays invisible to front desk. Submissions are timestamped, signed, and kept on the customer's record; clinical notes support amendments with a full history.

💡 Tip: keep front-desk forms short and public, and put clinical detail in a separate private form — the customer fills both, but each role sees only its own.
